A License Certification is used when a Nevada-licensed dental professional is applying for licensure in another state and needs the Nevada State Board of Dental Examiners to send official verification of the Nevada license directly to that state board.
Nevada law authorizes the Board to maintain official licensure records, including license and renewal information, and Board forms include an Exam / License Verification Order Form for this purpose.
When to Request a License Certification
Request a License Certification if you hold a Nevada license and another state board requires official confirmation of your Nevada licensure as part of its application process.
This process is for existing Nevada licensees who are using their Nevada license to seek licensure in another jurisdiction.
How the Process Works
To request a License Certification:
- Complete the License Certification Request Form locate on the Applications | Forms webpage.
- Submit the form with any required information so the Board can identify your Nevada license and the state board that should receive the certification.
- Allow 7 to 14 business days for processing.
The Board will then send the certification to the requesting state board as part of your out-of-state licensure application process. If you are applying in multiple states, a separate request may be needed for each state board, depending on the receiving board’s requirements.
